C. Everett Koop National Health Awards2008 ApplicationsAny Program which believes it may be competitive may apply without having been nominated. Nominations (as opposed to applications) are sought from all Health Project and Task Force members. Nominations need only include the name of the program, contact person, address, phone and fax numbers, and an email address if possible. Programs will be responsible for application development. The Program Nomination Task Force, will systematically develop additional nominations. We have added a category of "web-enabled" and hope to be able to make awards in this category. We would also like to receive applications from programs which have improved health and resulted in quantifiable productivity increases. Our goal is to be able to recognize any program of whatever type or source which can document true cost savings from health risk improvements and demand reduction. Previous winners are eligible except for the year immediately following the award and will be judged on the basis of all available documentation. However, to be considered, a previous winner must present and document some additional new data on their program's effectiveness and cost savings since the prior application. Categories (programs may be in more than one) are: chronic disease,
community, government, high-risk, innovator/vendor, insurance, integrated
systems of care, low birthweight babies, web-enabled, productivity enhancing
and worksite-based. Applicant Programs are required to send 15 copies of the application to the Program Selection Task Force Chair. Evaluation will be by the Program Selection Task Force, augmented by members of the Program Nomination Task Force, in three stages.
